Functions as a member of the multidisciplinary team to coordinate care with a focus on core measures compliance, zero defect goals, never events, patient education, and throughput; assumes accountability for achieving targeted outcomes and serves as the lateral integrator of care across disciplines. Demonstrates skill in systems management and effectively facilitates navigation of patients and families through the healthcare experience in collaboration with APN and other professional colleagues.

Requirements: Seasoned professional knowledge; equivalent to a Master's degree; knowledge in more than one discipline; Accredited School of Nursing. Two years of clinical experience in intermediate care, critical care, or cardiac nursing required. MS/CNL required; 3-5 years experience; Basic computer skills; Maryland Registered Nurse License; American Heart Association CPR Certification; ACLS; CNL. Hours are 8:30am to 5:00pm. Weekend coverage may be added at a later date.
