Evaluates patients clinically for placement both nursing and medically to ensure an appropriate level of care and a smooth transition from the acute hospital.

Requirements: Basic professional knowledge; equivalent to a Bachelor's degree; working knowledge of theory and practice within a specialized field; Nursing / RN; Five to seven years of experience.
